Photo and Video Release: I do hereby authorize RHA/REACT to utilize any and all photographs, pictures, video or other likeness of me or anyone assigned guardianship to me, as they deem appropriate in its promotional materials, website, social media platform or team films.

Player must have valid passport to travel out of country and will provide birth certificate upon request.

Liability and Team Event Release: In registering, the player understands that attending RHA/REACT Spring Program and using the facilities does so at his/her own risk. RHA/REACT and its owners, employees or agents, shall not be liable for any damage whatsoever arising from any personal injury or property loss sustained by participant with his/her family on or off the ice. Participants and parents assume full responsibility for all injuries and damages which occur in or about any programs on the premises, He/She/They does hereby fully and forever release discharged hold harmless RHA/REACT all associated facilities and its owner, employees, and agents from any and all claims, demands, damages or rights of action, present or future resulting from any person’s participation in any programs or use of the facility. In addition, he/she agree(s) to follow the rules of conduct and play set by RHA/REACT. Failure to do so may result in suspension from participation.

Privacy Policy: It is the policy of RHA/REACT that personal information, such as your name, personal postal and email address, or personal telephone number is private and confidential. Because your privacy is important to us, RHA/REACT maintains privacy policies to protect your personal information.

I agree to pay RHA/REACT registration fees in full by March 1, 2024. Deposit is due upon registration. I recognize that all payments are non-refundable.
